New Patients

We ask our new patients to download and fill out the following forms and bring the completed forms with you to your first appointment along with your insurance card (if applicable). We do require verification of insurance benefits prior to your first visit if we will be billing out for you.

Please contact our office 360-440-8060 and our staff will take care of this for you. You may also FAX all completed forms prior to your first visit, Attn: M. Jarrell 360-602-0895.

Arriving 5-10 min. prior to your scheduled treatment time will speed the intake process and allow for as much hands on therapy time during this initial visit. Please remember to allow for possible traffic congestion in order to arrive on time for your appointment. Please call if you are going to be late. Should you need to reschedule or cancel your appointment, please note that a 24-business hour- notice is required, in order to avoid cancellation fees.
